On this page you can download and print rosters, evaluations, and other forms needed to teach your students.

PASSWORD INFO

Current instructors may log in with their password.  

Usrer ID = first letter of your first name followed by last name (all lower case) example: John Smith =  jsmith

Password = the validation number on your certification card

If your password does not work, please request a password by clicking the following link:

1. Please include your name (first and last)

2. Please include the month and year you took your class

3. Please include the email address you would like your password sent 

please allow 24 hours for your password to be sent
